Farbood Nivi: Sure. Our Medical Director performs chart review on our mid-level staff. The NP and PA see patients and then have to have a percentage of their charts reviewed and signed off. As it stands, the system works fine for the actual review but there is no way for our director to indicate that he has reviewed the chart and "sign" it to prevent future edits to the notes for that appointment. Not all charts have to be reviewed. There would still possibly be a need for future edits to the notes for that appointment but they would need to be made outside the notes that were signed. When the director signs the chart they would need to be able to make their own notes as well for that appointment that would show that they came from a different user than the provider that actually saw the patient. The director is paid on a per chart reviewed basis and the ability to report how many and which charts a director signed would help tremendously as well.
I think this may tie into a need for tiered level account access to the system. Admin, Doctors, Mid-level, Nurses, Billing, etc.

Agreed. In order for electronic documentation to be considered legal, it has be signed and edits are time stamped.
